Eloise Tweeten, Founder & Advisor

Eloise launched Tweeten Eldercare Advisors in late 2007 and still loves to meet with clients in person and find the best fit to match their needs. A “perfect storm” of life experiences navigating care options for her own loved ones led her to do this fulfilling work with seniors and their families. Her father, a fiercely independent family practice physician in Wisconsin, and a frail aunt in Tucson taught her much about the challenges of aging. Both lived to age 96!

Eloise and her team take a personalized approach to help navigate the assisted living process. One size doesn't fit all!  Her experiences as a human resources executive and operations manager have helped her recognize well-operated facilities with caring, well-trained employees.

Michelle Gleed, Sonoma County

One of Michelle’s first memories as a young child was of reading books with her 101 year-old great-great grandfather, “Gramps.” His joy and kindness sparked Michelle’s love of elders (and books!) that continues today. Service through compassionate understanding has been a guiding force in Michelle’s life. While growing up in San Diego, she recruited friends to visit eldercare facilities to foster cross-generational insight and delight. After college, she moved to Sonoma County and worked with multi-generational families at a Montessori preschool as teacher, and later, the school director. She also created Pick of the Litter Thrift Shop to provide stable funding for Forgottten Felines, which is now the longest-running Trap-Neuter-Return (TNR) organization and Spay/Neuter Clinic in the nation. After nearly thirty years of non-profit administration, Michelle was called to join our TEAm after her own experience finding assisted living for family members and a search for similar heart-centered work. She is thrilled to be serving Sonoma County elders and their families!

Evelyn Hayes and Nicole Ruggeri, Client Services

Evelyn or Nicole are likely to be your first point of contact with Tweeten Eldercare Advisors. They conduct initial phone interviews with callers, gathering the most relevant information about care needs, social needs, geographic parameters, medical needs, and cost constraints. You may just need a referral to in-home care agencies, or to other community resources, in which case they can provide this information directly. They will also schedule an in-person appointment with one of our advisors to gather more in-depth information that will help us identify the best options for your unique situation. They also follow up with families after placement to ensure satisfaction with the care provided.  Evelyn brings over 20 years of industry experience, including management roles in assisted living and dementia-specialized communities. Nicole’s background encompasses 20 years in healthcare including customer service, administration, medical assistant, and medical assistant instructor before joining our team.

Ann Peronetto, Marin County

Ann Peronetto is a compassionate and committed healthcare professional who has worked in Marin County for over 30 years. Before joining our team, she had worked primarily in Hospice Care in Clinical Leadership roles. She has also worked as a Case Manager providing support for patients with spinal cord and traumatic brain injuries. Ann thoroughly enjoys being engaged with her Community and especially being of service to individuals who are in need of finding personalized resources and care. In her free time, Ann enjoys spending time with her family and her pets, and enjoying the many hiking trails that Marin has to offer.  

Linda Conner, Facility Services

Linda supports the rest of our TEAm by regularly updating information on independent and licensed facilities throughout the North Bay. She gathers information about assisted and independent living communities, as well as licensed care homes, that includes availability, resident demographics, and ability to care for specific resident needs. She records which communities can and cannot accommodate needs such as dementia, skilled services (e.g. injections), hospice, etc. She also monitors licensing reports and schedules client tours with our advisors. Linda is our longest-standing TEAm member, since 2018.
